Mexico IMSS family health fees rise to 20,600 pesos for older residents

IMSS family coverage rates rose in March

Mexico’s IMSS voluntary family health plan, Seguro de Salud para la Familia, updated its annual fees on March 1, 2026. The 60 to 69 age band now costs 20,600 pesos a year, up from prior rates that were closer to the 18,300 pesos figure cited in older references. Coverage still applies only inside Mexico and excludes people with preexisting conditions.

The program lets legal temporary and permanent residents enroll family members without formal employment. IMSS covers medical consultations, hospitalization, surgery, maternity care and medications, though some chronic conditions that need ongoing therapy remain outside the plan. Coverage starts on the first day of the month after payment.

Legal residents, not tourists, are the ones affected

The fee change matters most for expats and retirees with residency, especially older applicants who pay per person. A couple in their 60s would now pay about 41,200 pesos a year under the 60 to 69 rate.

Tourists and visitors on FMM visas still can’t enroll. Digital nomads without Mexican residency also need private insurance instead, since this IMSS option is limited to legal residents and screens out applicants with preexisting conditions.

How to apply and renew

Applicants can file in person at IMSS subdelegations or online through IMSS Digital, then complete a medical questionnaire. IMSS asks for ID, CURP, birth certificate and proof of address and renewals are due 30 days before the annual expiration date.
